Our Mission & Vision
To Build Capacity
Building capacity means supporting a participant to realise their full potential by being supported to overcome challenges, participate in and contribute to social and economic community life.
A Support Coordinator creates capacity building opportunities by understanding how to build positive connections within their broader community.
To Enable Choice
Support Coordinator enables choice and control by providing options of resources available, so a participant can make informed decisions about the supports and services they need.
To Connect Stakeholders
A Support Coordinator provides information to a range of stakeholders and/or networks who are connected to a participant, to facilitate the coordination of supports and services, so everyone is working towards the same goal.
A participant provides their consent for a Support Coordinator to act on their behalf, in these situations, which is part of their safeguarding role to ensure a participant receives the supports and services they agreed to. This may include supporting a participant to have a voice and negotiating the delivery of supports and services based on their individual needs.
Professional
Highly Trained Support Coordinators
The aim is for a Support Coordinator to wrap community, mainstream and funded options around people with disability, to offer and create opportunities to explore support and services, based on their individual needs and situation.
The aim is to:
● increase their connection and inclusion in community,
● easily navigate and access mainstream services, and
● source disability related supports using NDIS funds (funded service providers).
